In February 2026, the HealthChain project concluded, leaving behind three years of intensive collaboration between 33 organisations across seven countries and an impressive portfolio of nine digital health solutions deployed and clinically validated in five European regions.
HealthChain demonstrates the added value that digital technologies can bring to patients, clinicians and healthcare systems. In Croatia, the SAFE patient monitoring system achieved a 100% reduction in patient falls in hospital rooms where the solution was deployed, helping to improve patient safety and reduce preventable incidents. Another Croatian pilot, VERSO, enabled the remote monitoring of heart failure patients following hospital discharge, contributing to lower readmission rates and reduced mortality.
In Portugal, HealthChain supported the development of an AI-driven predictive model to help clinicians prioritise patients awaiting Transcatheter Aortic Valve Implantation (TAVI), supporting fairer and more efficient treatment decisions. The project also contributed to the structuring and standardisation of clinical data, providing healthcare professionals with improved access to information and enhancing clinical workflows.
Beyond these individual achievements, HealthChain generated lasting benefits for regional healthcare ecosystems. The project enabled SMEs to validate their solutions in real clinical settings, helping them scale their innovations and access new European markets.
Most importantly, HealthChain created a lasting community around healthcare innovation. Through its Community of Practice, the project brought together healthcare providers, SMEs, researchers and innovation organisations. This collaboration facilitated knowledge exchange, reduced innovation gaps between regions and accelerated the adoption of successful solutions across Europe.
Beyond the five core regions, an additional 15 regions joined the HealthChain Community of Practice, participating in interregional knowledge-sharing sessions on topics ranging from patient data management to artificial intelligence in healthcare. According to project partners, this network represents one of the initiative’s most enduring outcomes, providing a foundation for continued collaboration, reducing innovation disparities and offering a replicable pathway for the sustainable implementation of digital health solutions across European healthcare systems.
